Description Of Incident:

  •  After the web updates on January 2nd, members could not log into Online Banking.

 

Cause:

  • An old file (tiles-defs.xml) was used in the web updates for Jan 2nd. This outdated file did not contain information for the online access agreement. This issue caused members to receive a blank page after typing in their sign-on ID. The members never received their passmark image and therefore could not finish the login process.
  • The automated Online Banking monitoring process did catch this issue but notification did not go out.  The ITS Online Responder email distribution list was erroneously deleted at some point in the last few months.  

 

Resolution:

  •  Replaced the file with the most updated file from the source code management system.
