IT Change Management Forms

Attached are the 3 change forms we will be using going forward. They can also be found in the K:\IT Change Management\Forms directory. I will also be updating the "Change Management Procedures" with what I state below. 

We will now be using IslandTicket to store these forms and by hitting the submit button on the form will automatically open a ticket and attach the form. Below is a description of the flow.

If you have a production change that is major and will cause downtime you will be required to use the “Change Request Template”. When submitted this will open an IslandTicket and the help desk will then assign that ticket to you. Please use that ticket number and update the Change Management spreadsheet with that number. Once your change has been made please update the ticket with any notes or MIRs and close.

If you have a firewall change request please use the “Firewall Change Template” Once submitted the Help Desk will assign the ticket to Terry. Terry will get approval from your manager via IslandTicket for the change so signatures are no longer needed. 

For all other user account requests including VPN, Access to shares, OWA, access to a blocked website, etc. They must use the "User Change Request Template" I will send out an email to all staff explaining this and make sure it is easy to access from IslandNet. I have removed the signature field so whomever the owner of the ticket is responsible for getting authorization and justification via an IslandTicket email from the persons manager unless the request came directly from the manager. Help Desk will look at the change request and assign it to the appropriateperson. I don't think this form is necessary for new employees and we can continue to use Marie's tickets for those account setups. This will just be for changes.
