The Change Request template has been converted to a PDF form. At the bottom of the form is a Submit button. This will send the form to the whole ITS department so everyone is aware of future changes.
The process for changes will now be to add your change to the spreadsheet that we’ve been using in the past.
If your change was generated via an Islandticket request and requires no downtime then you will not be required to use this form just make sure to reference the Islandticket number on the spreadsheet.
If your change was not generated by an IslandTicket or requires down time, please use this form and click the Submit button.
Jason will assign a number to the request for tracking and auditing purposes.
