OSI - MasterCard Acceptance Form is Blank

Issue:

When I pull up the Mastercard Acceptance Form in OSI it is blank - no North Island Graphic, questions or checkboxes just the member information.

I tried thru the account as well as going to Print (on top) \ and selecting Print Group: Consumer Loan Servicing \ MasterCard Acceptance \ Selected and got the same thing

Resolution:

Click on My Forms \ Refresh Cache to clear the Form Cache

Log out of OSI and back in (no Re-Logon)
